Please contact me directly to discuss fees. I do not accept insurance, so I am private pay or out-of-network only. I will provide you with a “superbill” that you can submit to your insurance company for possible out-of-network reimbursement. It is your responsibility to confirm your coverage, including your out-of-network deductible.

The No Surprises Act is designed to increase price transparency and reduce unexpected medical bills. It requires healthcare providers to provide clients with an expected charge before services are provided.

Your rights under the No Surprises Act:

• You can request a Good Faith Estimate (GFE) explaining the total expected cost of non-emergency services.
• The estimate must include related costs, such as tests, prescription drugs, equipment, and hospital fees.
• Providers must give the estimate in writing at least 1 business day before your service or upon request.
• If your bill is $400 or more above the GFE, you may dispute it.
• Keep a copy or photo of your GFE for your records.
